First Visit Requirements

For your first visit, please bring:

  • Government-issued ID
  • All current medications in original pill bottles
  • Physician or radiology reports/CDs that you have
  • Primary Care Provider contact information
  • Completed New Patient forms

Opioid/Suboxone® Therapy

If you anticipate opioid or Suboxone® therapy, please review our informed consent and policies. Note: opioids are rarely prescribed at the first visit.

Appointment Guidelines

  • Arrive 10 minutes early to complete paperwork
  • Appointments start on time
  • All appointments must be scheduled 2+ hours in advance
  • No walk-ins accepted
  • $150 “No-Show” Fee Applied For:

  • Medication Safety: Bring all medication bottles or updated list to every appointment
  • Refills: Schedule next appointment in time for necessary refills – this is your responsibility
  • Behavior Policy: Rude or inappropriate behavior results in immediate discharge
  • Drug Screening: Urine drug screens may occur at any time for safety
  • Communication: We will communicate with your referring and primary care providers
  • Self-Pay: Payment expected prior to visit start
